'A Family Tradition of Caring'
The tradition of providing a full continuum of health care for individuals of all ages began in 1973 when the Tack Family welcomed Sugar Creek Rest's very first resident. Today, the Tack family dedicates three generations to quality long-term care.

One of the greatest benefits Sugar Creek Rest offers is peace of mind. Rest assured that you and your loved one will benefit from a broad range of professional medical services that include rehabilitative nursing care, Alzheimer's specialty care, sub-acute ventilator management , personal care services and independent living options.

The Quality Care Team Members of Sugar Creek Rest are committed to providing a wide range of services in a caring and professional manner. Quality care in a homelike environment is the hallmark of our extended family.

Dedicated to Delivering Quality Care

The staff of Sugar Creek Rest believe in the importance of maintaining quality care for each resident. Quality Care means providing services to the entire person - meeting the mental, physical and emotional needs of each resident in a personalized manner. Each staff member is dedicated to providing care, concern, compassion and comfort to every resident and their family.

Our comprehensive, restorative programs and recreational activities are second to none. Our residents enjoy a range of fun, exciting activities including an annual carnival and regular jaunts to ice cream parlors and the local shopping mall.  Weekly nondenominational worship services and our in-house chaplain cater to the spiritual needs of each resident.

Home Away From Home

Sugar Creek Rest is nestled in the heart of rural Armstrong County on the Tack family farm. Rolling green hills, beautifully colored leaves and a picturesque lake with swans and geese complete the picture - perfect scene.

At Sugar Creek Rest, we know the importance of having a special place to call home. Sugar Creek Rest offers semi-private and private rooms with wall-to-wall carpeting and brightly-colored, cheerful decor. Cable television and telephone outlets are available in every room for your convenience. Residents are also encouraged to bring cherished possessions and mementos to give their room that added personal touch.
