What is Hospice Care?
Hospice care is specialized healthcare designed for individuals with a life-limiting illness when the focus shifts from curative treatment to comfort, symptom management, and quality of life.
Our hospice team provides physical, emotional, social, and spiritual support while helping patients remain as comfortable and independent as possible. Care can be provided wherever a patient calls home, including private residences, personal care communities, assisted living residences, and skilled nursing facilities.
Hospice also provides support for family members and caregivers, ensuring they never have to navigate this journey alone.

A Team Approach to Hospice Care
Hospice Medical Directors & Physicians

Our Hospice Medical Directors provide clinical guidance and oversight to ensure patients receive the highest quality care. At the same time, patients may continue working with their existing Primary Care Physician, who remains actively involved in managing their care and treatment decisions.
By working collaboratively, our medical team helps ensure seamless communication and personalized care.

Registered Nurse Case Managers
Registered Nurse Case Managers coordinate and oversee each patient's individualized plan of care. They monitor symptoms, provide education and support to caregivers, coordinate services, and help ensure patients remain comfortable throughout their hospice journey.
Our nurses are available 24 hours a day, 7 days a week to provide guidance, answer questions, and address urgent concerns when they arise.

Licensed Practical Nurses (LPNs)
Our LPNs work alongside Registered Nurses and other members of the care team to provide treatments, monitor patient needs, and carry out the individualized plan of care. Their compassionate, hands-on support helps promote comfort and positive outcomes for patients and families alike.

Home Hospice Aides
Home Hospice Aides assist with personal care needs such as bathing, dressing, grooming, and hygiene. These services help patients maintain dignity and comfort while easing some of the day-to-day responsibilities placed on family caregivers.
Emotional, Spiritual & Family Support
Hospice care extends beyond physical needs. We recognize that serious illness affects the whole person—and often the entire family.
Medical Social Services
Our social workers provide guidance, advocacy, and support for patients and families facing difficult decisions. They help navigate healthcare options, connect families with community resources, and provide emotional support during challenging times.
Spiritual Care

Medical Equipment, Supplies & Medications
To help reduce stress and improve comfort, medical equipment, supplies, and medications related to the hospice diagnosis are provided and coordinated through Three Rivers Hospice.
This allows families to focus on spending meaningful time together rather than managing logistics and healthcare equipment.
Respite Care for Family Caregivers
Caring for a loved one can be rewarding, but it can also be physically and emotionally demanding. Hospice respite care provides temporary relief for caregivers, allowing them time to rest and recharge while ensuring their loved one continues to receive quality care.
