Our team of Medical Directors provide guidance and support for our hospice team.

Primary Care Physicians
Your existing Primary Care Physician continues to lead and manage your care. Our clinical team works hand-in-hand with your physician as they monitor your plan of care.

Registered Nurse Case Managers
A registered nurse will coordinate and provide your care needs along with providing education to your caregiver. In addition to regular visits, our nurses are available 24/7.

Licensed Practical Nurses
The LPN works under the supervision of the nurse to provide the plan of care and treatments, working collaboratively with the members of the team to meet positive care outcomes.

Home Hospice Aids
Under the supervision of the RNs, Aides will assist you with your personal care such as bathing and dressing.

Medical Social Services
Based on specific needs, social workers are available to provide guidance to you or your family with healthcare decisions, and help you navigate the mental, emotional and social challenges you are facing.

Caring Volunteers
Our volunteers provide a range of support services to patients and families, such as companionship and respite for their caregivers.

Our chaplains provide spiritual and emotional support regardless of your denomination.

Bereavement Support
Whether young or old, losing a loved one is never easy. Our bereavement counselors provide grief and loss counseling, support groups, workshops, social outings, and memorial services. Our support does not end with the loss of a loved one.

Medical Equipment & Supplies
Medical equipment, Medications, and Supplies related to the life-limiting diagnosis are provided and paid for by Three Rivers Hospice.

Respite Care
Respite care is short-term relief for hospice caregivers. Hospice respite care allows the caregiver to rest and recover to continue supporting a loved one at the end of life.
