The History of Quality Life Services

1956

It all started with a family, a farm and a dream


In 1956 Ken and Nancy Tack purchased an old farm that would grow into a home for their family and would eventually become Quality Life Services. The farm is the site of the flagship nursing home, Quality Life Services - Sugar Creek. Four years later a 60-bed skilled nursing addition was built on the property, followed by the construction of an independent living community, Armstrong Retirement Village. The current Personal Care Home is the site of the original nursing home. 

1990's


In the early 1990s ownership of the company switched from the first generation to the second – passing ownership onto brothers, Ken and Phil Tack.

Years of Growth for Skilled Nursing and Memory Care


  • In 1992 they acquired Quality Life Services Mercer, then known as Countryside Convalescent Home. The building includes a unique memory care neighborhood with a town square atmosphere and café dining. 


  • In 1995 Quality Life Services Sugar Creek added sub-acute and memory care wings to the building. This was also the year that Quality Life Services was formed and the corporate office began operations on the top floor of Sugar Creek Rest. 


  • In 1999 the Tack family added Quality Life Services Chicora, then known as Chicora Medical Center to the portfolio of homes. The facility is located in the heart of Chicora. 

2000's


Quality Life Services Grove City joined the Quality Life Services family in 2001. One year later the home underwent a major renovation to add a personal care and specialized memory care neighborhood. In 2003 Independent living apartments were added, creating the first and only Continuing Care Retirement Community in Grove City.

New Initiatives and a New Generation Takes Ownership


  • In 2005 Don’t Stop Dreamin’ was formed, starting out as “Dream a Little Dream”. The goal of this non-profit organization was to provide meaning, fulfilment and fun to the lives of residents in the nursing homes. 


  • In 2006 Quality Pharmacy, a retail and institutional pharmacy located on the Chicora campus opened. This pharmacy continues to serve the general public, a number of other local care homes and provides for the pharmaceutical needs of all of the Quality Life Services homes and residents.


  • By 2006 the Quality Life Services Corporate Office had outgrown the upper floor of Sugar Creek Rest and needed a new space to operate. The company moved to their office in downtown Butler and still occupies the space.

  • 2007 brought about the most transformative shift to date – the transition of ownership from the second generation to the third generation. Current owners Steven Tack, Susie Tack-Beardsley and Mary Susan Tack-Yurek took the place of their fathers as owners and operators of Quality Life Services. 

  • 2007 also brought about the addition of two more nursing homes to the Quality Life Services Family:  Quality Life Services Sarver and Quality Life Services Markleysburg.

2010's


2010 was another exceptional year for Quality Life Services with the acquisitions of both Quality Life Services Apollo and Quality Life Services Henry Clay.

Collaborative Care:  Family Home Health, Hospice and More


  • In 2012 Don’t Stop Dreamin’ was granted official 501c3 non-profit status.  Click here to learn more!

  • Quality Life Services New Castle joined the Quality Life Services Family in 2014. This facility is the largest solely skilled nursing home within Quality Life Services.

  • In 2015 Quality Life Services Westmont joined the company.

  • As time went on, the goal for Quality Life Services grew to meet the needs of people in all post-acute settings. The first step in providing more extensive, collaborative care, was acquiring Family Home Health and Three Rivers Hospice in 2017. 

  • In 2018, Quality Medical Associates was founded. This organization employs a physician and nurse practitioners who work at the nursing homes to better serve the needs of the residents.

Adapting to Change as We Plan for the Future

The pandemic in 2020 changed the landscape of healthcare and staffing for healthcare facilities. In response to those changes, Quality Staffing Solutions opened in 2022. This staffing agency provides a pool of willing, competent staff who will travel to multiple locations to support the in-facility staff and meet the needs of the residents. This model ensures that all of our staff are receiving the most relevant training and providing the best care to our residents and patients.
