Skilled Nursing & Rehabilitation

Medical and rehabilitative services to help patients recover and thrive to the best of their abilities. This is usually recommended after an acute hospital stay. The goal is a short term stay then back home. Most stays are covered by insurance.

Assisted Living / Personal Care

Services for those who need assistance with bathing, dressing and medication management. We offer supervision with all care needs. Independent apartments / home rentals are available in some locations. Private pay only, commercial insurance does not cover the stay.

Long Term Care

For those who need 24 hour care, requiring assistance or complete care with dressing and bathing. Includes help with long term medical needs. Private pay or medical assistance only. 

Memory Care

A designated area offering security and compassionate care for those who have a tendency to wander. Providing specialized services for patients with Alzheimer's or other forms of dementia. Private pay or medical assistance only.

Home Health

Family Home Health provides in home medical, rehabilitative and social services. Some services may be available in a personal care setting. Most services are covered by insurance.

Hospice

Three Rivers Hospice offers end of life care in a patient's home or in a personal care or nursing home setting. This service is covered by Medicare and most insurances.
